Appointment Reminder and Confirmation Message Templates for Clinics

Eleven copy-and-paste WhatsApp and SMS templates for clinics: booking confirmation, 48-hour and same-day reminders, rescheduling, no-show follow-up, recalls, waitlist offers and review requests, with notes on template approval and consent.

Most clinic reminders are written once, years ago, by whoever was on the desk that day. They are too long, they do not ask for a reply, and they give no way to reschedule except "call the clinic". Below are eleven templates that fix that, written for WhatsApp first (they also work as SMS), with the rules for getting them approved on the WhatsApp Business Platform.

Copy them, replace the bracketed fields, and keep the structure.

Key takeaways

  • Every reminder should answer four questions in order: who is this from, when and where, what do I do now, how do I change it.
  • Ask for a reply. "Reply 1 to confirm, 2 to reschedule" turns a reminder into a commitment.
  • Keep clinical detail out. Logistics only.
  • On the WhatsApp Business Platform, anything you send more than 24 hours after the patient's last message must be an approved template. Write them as "utility" messages.
  • Add a consent line the first time, and an opt-out.

The rules behind a good reminder

  1. Identify the clinic in the first line. Patients get messages from unknown numbers all day. "Hi Aisyah, this is Eversmiles Dental" is enough.
  2. Put the appointment details on one line. Day, date, time, doctor. Add the address or a map link for new patients.
  3. Ask for one action. Confirm or reschedule, with a one-character reply. Do not ask three things.
  4. Make rescheduling easy. If the only way to change the appointment is a phone call, you will get no-shows instead of reschedules. The no-show guide explains why.
  5. Keep it short. Under 300 characters for the reminder itself. Details like what to bring can go in the confirmation message, not every reminder.
  6. Match the patient's language. A reminder in a language the patient reads easily gets acted on. One they half-understand does not.

The templates

In each one, the bracketed fields are what you fill in. On the WhatsApp Business Platform they become numbered placeholders like {{1}}.

1. Booking confirmation (sent immediately)

Hi [Name], this is [Clinic]. You're booked with [Doctor] on [Day, Date] at [Time]. Address: [Address or map link]. Please bring your ID and any previous reports. Reply here if you need to change anything.

2. Reminder, 48 hours before (confirmation request)

Hi [Name], this is [Clinic]. Reminder: your appointment with [Doctor] is on [Day, Date] at [Time]. Reply 1 to confirm or 2 to reschedule.

This is the most important message in the set. It is the one that moves the no-show rate.

3. Reminder, same day

Good morning [Name], see you today at [Time] with [Doctor] at [Clinic]. Reply 2 if you need to reschedule.

4. Reschedule offer (after a "2")

No problem, [Name]. Here are the next available times with [Doctor]:

  1. [Day] [Time]
  2. [Day] [Time]
  3. [Day] [Time] Reply with the number, or tell me a day that suits you.

5. Unconfirmed, morning of

Hi [Name], we haven't heard back about your [Time] appointment today with [Doctor]. Reply 1 if you're coming, or 2 to move it. We'll hold the slot until [Cut-off time].

6. No-show follow-up (same day)

Hi [Name], we missed you at [Clinic] today. No problem at all. Reply here and we'll find you a new time with [Doctor].

Send this the same afternoon. The rebooking rate drops sharply after 24 hours.

7. Waitlist slot offer

Hi [Name], a slot has opened with [Doctor] on [Day, Date] at [Time]. Reply YES in the next 30 minutes to take it, otherwise we'll offer it to the next patient.

8. Post-visit check-in (next day)

Hi [Name], this is [Clinic]. How are you feeling after yesterday's visit with [Doctor]? If anything is worrying you, reply here and the team will get back to you.

Keep this one open-ended and clinical-free. If the patient reports a problem, a person should answer.

9. Recall (routine review due)

Hi [Name], it's been [6 months] since your last [cleaning / review] at [Clinic]. [Doctor] recommends a check-up around now. Reply BOOK and we'll send you some times.

10. Review request (sent only to patients who had a good visit)

Thank you for visiting [Clinic], [Name]. If you have a moment, a Google review helps other patients find us: [Link]. Thank you from [Doctor] and the team.

11. After-hours auto-reply

Thanks for messaging [Clinic]. We're closed right now and open at [Time] tomorrow. If you'd like to book, reply with the service you need and a preferred day, and we'll confirm first thing. For emergencies, please call [Emergency number].

If an AI front desk is answering the number, you do not need this one; it books the appointment there and then.

Language variants

The 48-hour reminder in the languages clinics in Singapore, Malaysia and India most often need. Have a native speaker on your team check them against your house style.

Bahasa Melayu

提醒您:您与 [Doctor] 的预约时间是 [Day, Date] [Time]。回复 1 确认,回复 2 改期。

ClinicOS sends reminders in whichever of English, 中文, Bahasa, हिन्दी or தமிழ் the patient last wrote in, so you do not need to maintain separate lists.

Getting templates approved on WhatsApp

If you send reminders through the WhatsApp Business Platform rather than from a phone, every message you initiate outside the 24-hour window must be a template Meta has approved. Three things get templates through review:

  • Category: Utility. Reminders, confirmations and reschedules are utility messages. Do not mix in promotions.
  • Clear placeholders. Each {{n}} must have a sample value when you submit, and the message must make sense without the placeholders.
  • No vague or promotional phrasing. "Don't miss our offer" will be rejected or reclassified as marketing, which costs more per message.

Templates 1, 2, 3, 5, 6, 7, 8 and 9 above are written to pass as utility messages. Template 10 is marketing under Meta's rules and should be submitted as such. Our WhatsApp booking guide covers the Platform setup.

The first time you message a patient about an appointment, include a line that covers PDPA in Singapore and Malaysia and the DPDP Act in India:

We'll send you reminders about your appointments on WhatsApp. Reply STOP at any time to opt out.

Honour the STOP. Keep the record of consent with the patient's file, not on someone's phone.

Timing summary

Review request 1 to 2 days after a good visit
Recall When the review is due, with a booking prompt

Frequently asked questions

How long should an appointment reminder message be?

Under 300 characters. Clinic name, appointment details on one line, and a single reply instruction. Put what-to-bring and directions in the booking confirmation, not in every reminder.

Should reminders ask the patient to confirm?

Yes. A reminder that asks for "1 to confirm, 2 to reschedule" turns a passive message into a commitment and tells your front desk who still needs chasing. It is the single biggest lever on no-shows.

Do I need WhatsApp template approval to send reminders?

Only if you send through the WhatsApp Business Platform (API). Messages you start more than 24 hours after the patient's last message must use an approved template. From the free Business App on a phone there is no approval step, but also no automation.

Get consent the first time you book a patient and offer an opt-out. Appointment reminders are generally treated as service messages rather than marketing, but a clear consent line and an honoured STOP keep you aligned with PDPA and the DPDP Act and avoid complaints.
